I give my consent for those in my family (listed above) to participate in Zachariah’s Acres family event, including but not limited to games, crafts, snacks, wagon rides and other activities. I, on behalf of myself and/or my family, hereby release and hold harmless Zachariah’s Acres Inc., and their officers, directors, volunteers and employees from any injuries, claims, damagers or liabilities of any kind whatsoever that I or my family may suffer or incur while involved in activities.

I further acknowledge and agree that participating in this event may result in publicity. The participating child(ren) and his/her parents or legal guardian hereby irrevocably authorize Zachariah’s Acres, Inc.: (a) to publicize and use the Recipient's likeness, voice and features, with or without his/her name, for any publication, promotion, trade or business use, or any other purpose; (b) to photograph, videotape, film and record each child.
